Application Requirements
When applying for a Create Healthy Scholarship, please submit the following:
- Completed Application (Click here to view instructions for applying)
- Short Answer Prompt: In a paragraph, please explain how this scholarship will help you achieve your aspirations in the healthcare field?
- Transcript or Proof of Enrollment
- Proof of enrollment or acceptance into a program.
- Letters of Recommendation
- Please submit two letters of recommendation: one from current employer and one from current colleague.
- Proof of Financial Need (e.g., FAFSA Student Aid Report, recent tax return, or a brief written explanation).
- Be enrolled in courses that are in the healthcare field (see attached for listing of areas of study).
- Be a U.S. citizen or permanent resident.
- Be a resident of or employed by an organization in Gillespie County, Blanco County, Llano County, Mason County, or the town of Comfort.
- Be enrolling in a degree or certificate program.
- Be one of the following:
- Working professional returning to school, GED recipient, career-changer, student with unique educational journey
Preference will be given to those with proven financial need.
Selection Criteria | Adult Learners
Adult learner recipients will be selected based on the following criteria:
- Letters of recommendation (one from current employer and one from current colleague)
- Response to short answer question
- Proven financial need
