When applying for a Create Healthy Scholarship, please submit the following:
- Completed Online Application
- Short Answer Response: In a paragraph, please explain how this scholarship will help you achieve your aspirations in the healthcare field?
- Official High School Transcript
- Letter of Recommendation
- Please submit one letter of recommendation from someone who knows you well academically or professionally and can speak to your strengths, character, and achievements
- Proof of Financial Need (Recent tax return, FAFSA, optional statement of financial need)

Applicants must:
- Be a high school graduate of 2026.
- Must reside in or attend a high school in Gillespie County, Blanco County, Llano County, Mason County, or the town of Comfort, TX.
- Be enrolled in courses that are in the healthcare field (see attached for listing of areas of study).
- Be a U.S. citizen or permanent resident.
- Be enrolling in a college, university, trade school or certificate program within the healthcare field
Preference will be given to those with proven financial need.
