When applying for a Create Healthy Scholarship, please submit the following:
- Completed Application
- Short Answer Prompt: In a paragraph, please explain how this scholarship will help you achieve your aspirations in the healthcare field?
- College Transcript or Proof of Current Enrollment.
- Documentation of GPA of at least 2.5
- Proof of Financial Need (e.g., FAFSA Student Aid Report, recent tax return, or a brief written explanation).

Applicants must:
- Be currently enrolled in courses that are in the healthcare field (click here for list of areas of study).
- Be a U.S. citizen or permanent resident.
- Be a resident or former resident in Gillespie County, Blanco County, Llano County, Mason County, or the town of Comfort.
- Be enrolled in a degree or certificate program.
Preference will be given to those with proven financial need.
